Care after Discharge

There are occasions when patients need additional treatment or care after they are discharged from the hospital. St. Mary’s Health Care System offers a complete continuum of care to meet these needs.

Center for Rehabilitative Medicine

Located on the 5th floor of St. Mary's Hospital, the Center for Rehabilitative Medicine provides intensive, inpatient rehabilitation and nursing care for patients recovering from a wide variety of debilitative conditions and illnesses, including stroke, certain major surgeries, Parkinson's disease, and more.

Outpatient Services

Offering a wide variety of outpatient services from radiology and diagnostics to rehabilitation, St. Mary’s provide patients with convenience, quality care and the latest technology to fit into their active lifestyles.

Home Health Care/Hospice

St. Mary’s has a long-standing tradition of providing quality, compassionate care with dignity and respect both in the traditional hospital setting and in homes across Northeast Georgia. Home Care and Hospice services include home health care, Telehealth, home hospice care and St. Mary’s Hospice House.

Care Alert

St. Mary’s Care Alert allows individuals to summon help from their home with the push of a button. Our emergency alert system is monitored 24 hours a day, seven days a week by trained professionals.

Infusion/Wound Care

St. Mary's Ambulatory Infusion and Wound Care Services help patients manage treatment, maximize health and avoid inpatient hospital stays. Patients suffering from a wide range of conditions such as anemia, asthma, Crohn's disease, immune deficiencies, rheumatoid arthritis and non-healing wounds receive care from experienced registered nurses in a comfortable healing environment.
